cardiac echinococcosis accounts for fewer than 2/100 of hydatid disese, yet concentrates two feared perioperative catastrophes into one operation : parasitic anaphylaxis from antigen spillage, a distributive shock treated with epinephrine and volume; and cardiogenic of obstructive collapse from a mass lesion contacting the myocardium and croronary circulation, in which volume may be futile or harmful. both present as sudden hypotension under perioperaive conditions that remove the cue normally used to tell them apart. the surgical literature is well developed; the anesthetic literature consists of isolated case reports. this narrative review examines the immunology and true incidence of parasitic anaphylaxis, its recognition and treatment on cardiopulmonary bypass, the physiology of a compressive extracardiac mass, the discriminating role of transoesophageal echocardiography,the choice of perfusion strategy and the overlooked systemic toxicity of scolicidal agents .
